Any 'Requests to bid' must include (fax or email) a qualification statement listing but not limited to past work,
references, & up to date business licenses and County registrations, including State of Florida annual report.

We also need a sample insurance Certificate showing current policies with coverage amounts
(General Liability, Worker's Compensation & Auto). A copied sample certificate is acceptable but original certificates are required if awarded work.

Please note, our Subcontractor Contract or other Project documentation may require additional insured listings
in addition to "JJW" being the certificate holder.  All Vendors must be in compliance with State and local statutes throughout completion of the project.
